If you feel you are a victim of personal ID theft, and the ID thief opened a new account, you will want to ensure that you are not made responsible for the debt incurred. The FTC provides an excellent resource with the ID Theft Affidavit, found here: FTC ID Theft Affidavit

Complete the affidavit and forward to your creditors as soon as possible (the affidavit is optional for companies, first contact the credit company to verify they will accept the FTC ID Theft Affidavit). Some companies may request additional information.
